Meet Frank Hermantin, MD

Orthopedic Surgeon and Longevity Medicine Specialist

With over 25 years of experience as a board-certified Orthopedic Surgeon and fellowship-trained spinal specialist, I bring deep clinical insight to the Liminal Longevity team. My years in orthopedic trauma surgery have only reinforced a truth I’ve seen throughout my career: many of the consequences of aging poorly such as spinal degeneration, fractures, and mobility loss, are often preventable. I’ve treated countless patients facing these challenges, and I’ve seen firsthand how much could be avoided with stronger muscles, better balance, and healthier metabolic function. That’s what led me to expand my practice into proactive, regenerative, and preventative care.

I’m now certified in Longevity Medicine and hold a Certificate of Added Qualification in Hyperbaric Medicine, which allows me to apply a broader toolkit to help patients build strength and resilience for the long term. At Liminal Longevity, I focus on helping people stay strong, mobile, and active through evidence-based strategies rooted in orthopedic science, regenerative therapies, and functional wellness. I treat every patient the way I would treat my own family, with honesty, precision, and a long-term vision for health. I’m also fortunate to work alongside my wife and co-practitioner, Dr. Jennifer Phelps, a double board-certified physician in Integrative and Family Medicine with over 25 years of experience. Together, we offer a truly comprehensive approach to longevity merging structural, metabolic, and lifestyle medicine to help you live stronger, longer, and happier.

I’m deeply passionate about our mission at Liminal Longevity: helping people live with vitality, strength, and purpose at every stage of life. For more than 25 years, I’ve had the privilege of being present as people step into new moments of health, transformation, and becoming. That depth of human experience, paired with cutting-edge longevity science, is the foundation of a new paradigm in medicine, and one Dr. Hermantin and I are proud to share.

My path has been shaped by curiosity and a commitment to whole-person health. Prior to medicine, I studied biochemistry, pharmacology, and toxicology, a foundation that deepened my practice in functional medicine and, now, longevity medicine. I’m board-certified in Family Practice and Integrative Medicine, with advanced certifications in Longevity Science and Hyperbaric Medicine. Advances in genetics, technology, and sophisticated diagnostics are redesigning what medicine can offer, and this is the science we bring to every patient.

What makes Liminal Longevity distinctive is how we hold that science alongside deep attention to mental and emotional health. I’ve had the honor of holding space for communities in their darkest hours: after natural disasters, mass shootings, and war. That depth travels with me into every room, shaping how I selectively use trauma-informed, mind-body medicine and Ketamine-Assisted Psychotherapy, in both individual and group settings. More than anything, I want every person who walks through our doors to feel seen, safe, and gently held in their becoming.

I’m grateful to do this work beside my husband and co-founder, Dr. Frank Hermantin, a board-certified orthopedic spine surgeon. We are joined by our son, Pharo Hermantin, who brings both business acumen and a lifelong dedication to physical training. As a family-run practice, we hold a core belief that community is often the medicine, and we are committed to creating spaces where people can live stronger, longer, and more fully, both individually and collectively.

Business Manager and Certified Personal Trainer

With a lifelong passion for fitness and health, I’ve dedicated myself to exploring innovative ways to optimize human performance and well-being. My personal journey, marked by both discipline and challenge led me to discover the transformative power of hyperbaric oxygen therapy, which now plays a central role in my wellness philosophy. A graduate of Leeds Business School with a focus on business development and entrepreneurship, I bring strategic insight together with hands-on expertise as a NASM-certified trainer.

I work hand-in-hand with our lead physicians to review patient charts and leverage comprehensive diagnostic data including labs, imaging, and biometrics to design truly personalized fitness plans. Each program is dynamic, evolving alongside the patient’s progress to ensure alignment with their long-term health goals and physiological changes. Driven by curiosity and a commitment to innovation, I continuously seek out cutting-edge sports science treatments to support longevity, resilience, and total-body vitality.
